Folder with Question Mark

My Macbook Pro 13" Mid 2011 is showing Folder with a question Mark at start up.  I have connected using Command R to access Internet Recovery mode but cannot seem to get anywhere.  Does anyone have step by step directions on how to use this method of recovery??

 

I have worked on this MB for a long time and would like to get this behind me.

    Select Install OSX while in Recovery.

     

    Cmd R = Recovery

    Opt-Cmd R = Internet Recovery

    If it only shows the folder with the questionmark at startup time, that means that it can't locate the drive with the OS on it.  Often, this indicates that the drive is having some serious errors, but it it possible that it is only a few corrupt files.  Running the Disk Utility form the Recovery partition or Internet Recovery should help you to tell how severe the issue really is.  You may need to replace the HDD to resolve this.
