Does the call reach the phone, but not ring, or does it not reach the phone? Usually this means that your line is set up to block unknown callers; this is a feature of your carrier, not your phone. So start by contacting your carrier.

The call reaches the phone, is displayed, but neither rings nor vibrates despite my settings. I have attempted the reboot, cycling of Bluetooth (not sure why some thought that was related), and checked my Do not disturb settings with no luck.

That sounds like a Do Not Disturb issue. DND will only ring on calls that are in your Favorites. By definition "unknown" cannot be a Favorite. Try disabling DND completely. Settings/Notifications/ Do Not Disturb - turn Scheduled to OFF to completely disable it.

Unfortunately, my DND is off and has been off. I never use it (i.e. never put it On). Further, the DND is not Scheduled (i.e. off).
Was there a recent software change that has a glitch?

I have not and admit my infamiliarity with such steps. I recognize that Google can get me there but if you have a link that provides me safe steps, I'm game. As a second, any reason that I might want to avoid Restore or Restore as New? I.E. will it delete contacts, music, etc?

Reset: Hold HOME and SLEEP buttons at the same time until an Apple logo appears. This will take about 10 seconds, and you will not lose any data. It essentially reboots iOS.
If this fixes the problem stop here.
Restore: Connect to your computer. When the iPhone appears in iTunes click on its name, then click on the Restore iPhone button. This will back up your phone, then reinstall iOS and restore the backup that was just created.
Restore as New is the same, but when prompted choose to set up the phone as New.
