However, I am unsure if a SIM card was pre-installed in it. I bought it brand new. Is it customary for new iPads of my particular make/model/year to have a SIM card pre-installed in it?

It may also be as simple as removing and reinstalling the SIM.
Is the iPad in question Wi-Fi only? If so, there is no SIM slot.
If it is a 3G/4G iPad, a SIM is required to access the cellular network.

Has it ever worked? - YES. I browse the internet quite often.
Is there a SIM in the device? - I don't know. I personally did not install one in the device. However, I am unsure if a SIM card was pre-installed in it.
Have you tried removing and reinstalling the SIM? - NO. I dont feel comfortable doing this. I will just make an appt with an Apple associate at my nearest Genius Bar.
Has anything resembling troubleshooting been attempted? - Not relative to this matter. I visited the Genius Bar to help me with setting up the iPad after I purchased it.
