Have you tried the headset with someone elses phone? This might help determine if the headset is defective.

Yes. It is the same headset I was using with my 4. I did not have any problems. I am hoping maybe there is a setting or something I might try?

The same happens to me on my bluetooth headset. After talking for 5 mins the other party cannot hear me suddenly but I can hear him/her clearly in bluetooth headset. Do you find a solution finally?
I have tested with my family's iPhone 5 and the same happens. But the bluetooth headset was working fine before with iPhone 4S.
Let's report this issue to Apple below together to raise the concern of this issue:

I have not found a fix for this and it is still happening. I will report it today for sure. I really need my bluetooth to work. Thanks for posting. I was afraid it was my phone and I didn't want to go through the hassel of exchanging it!

I have had this happen. I also keep having different headsets randomly lose the BT connection and have to be reconnected in the middle of a conversation.
