Q: What does "service battery" mean?

When I hold the Option Key & click on the battery-status icon, it reads "Service Battery". Does this just mean the battery is not holding a charge (which it really never has); or do I need to replace the battery to avoid damaging my laptop?

MacBook Pro, OS X Mountain Lion (10.8.2)

Eventually. The battery could start to swell or worse if not removed and properly disposed.
