Well you have covered everything I can think of
You could call AppleCare in your country and they may check the status on their database of your iPhone
Then you will know if it is you or an issue with the carrier processing the request correctly

Thank you Peter, didn't think of that. I can pop in to an apple store that is close to work tomorrow afternoon see how far I get. My Carrier(customer service) did confirm unlocking was approved.
