If it's registered to your account then you can login on this page and remove it, and she can then add it to her account : https://supportprofile.apple.com/
If it appears in your 'manage devices' section then you can also remove it from there (assuming that it's not still within the original 90 days of association) : http://support.apple.com/kb/HT4627
You can also log out of your account on the iPad by tapping on your id in Settings > iTunes & App Stores
