I guess, because I am on 4S, nobody cares. I don't see a fix coming any time soon.
Ah well. I will upgrade to the 5S around Xmas, and I will toss this 4S into the ocean.

You're not alone. This is really annoying for me.
The ONLY, reliable way I've found is to:
1: Never ever close the music app
2: Never start playing from the control centre
3: Before docking, open music app and ensure you go to 'now playing'
This seems to work, but if for any reason you do forget... You're screwed, unless you can remember where you left off.

even if you don't close the Control Center, it doesn't matter.
I play a few songs in the car, unplug the headphone, go work for a few hours, leave the music app open, tinker around with a few texts, some games -
I get back in the car a few hours later, plug the phone in to play -
but the control center's music app shows blank.
Then I open the App, and look - and guess where the song is? It's not in the last spot I played. It has not remembered AT ALL. It's somewhere else, it's way back somewhere else. So if I simply just click "now playing" and let it play - it's not the last song and place it played - it's some other song and place!
So, not only does the Control Center not remember, the App itself, inside the OS, doesn't remember, either!
So, I go down and pick the song I was actually last playing, and start playing again. If I continuously play from there, and do it all over again, without touching play again, going back out to the Control Center and going throught that routine again - it's somewhere else.

Yeah, I find the same with the control centre being blank after unplugging headphones and using phone for a bit.
However app seems to remember for me.
e.g. Last night, I unplugged my phone headphone from the car. I've played around with it all night. Plugged the charger in. Closed all apps except the music app.
This morning I unlock and the app has saved the exact position by going to 'Music >>> Now Playing' however I do find it very frustrating.
It would be great if it worked the same as before, ie open control centre and press play.
Post if you ever resolve, I'll do the same.

Sorry I mean to say that "even if you don't close the Music app"
which I've never ever closed, since the beginning! It's always running! Except for when I did the updates, I've never closed it.
I just think that it's Control Center. They have to fix that connection to the Apps.
Plus Apple just wants me to throw away my 4S into the ocean so I would buy the new 5S.

I have an iPhone 5, and I have the same problem. It is like that no matter if I play one artist, one song, shuffle all songs or playlist.
I also have the problem where I cannot go to the previous song! That is really annoying! Sometimes I want to listen to the song before, and not with the buttons, not with the headphones. I can tripple-click until I am blue in the face, and nothing! Just the song I am actually IN! Sometimes when I change the songs ON my actual phone, I click a bit too fast, and no matter how fast I click the back arrows, it will not go back to the previous song(s)! It is killing me!!!
