Deleting an App off an iPad is as simple as holding down on it until it wiggles and pressing the (x) badge that pops up in the corner of it.
That removes it entirely from the device.
If its coming back for some reason, then there's something else you are doing or not doing.
As Jim points out, if the App was synced to a computer running iTunes and then removed from the iPad, a future sync will put it back onto the iPad.
You would need to delete it from iTunes on the computer as well.

Feb 12, 2013 10:26 AM in response to Vdogrrlby Phil0124,The Purchased list is just that, a list of all your purchases, the App is not on your device anymore. Should you ever want to redownload you can do so from there and in the case of Apps you paid for you can do so for free.
You cannot delete Apps from that list as that would defeat its purpose of allowing you to download purchased apps without having to re-pay for them. Its apple's way of remembering what apps you already payed for and can download again, without charging you for them again.
You can however hide them if you wish:

Oct 28, 2013 2:48 AM in response to M Marcinikby King_Penguin,If you don't get the 'x' on any of the apps that you've downloaded (you can't delete built-in apps) then check that Settings > General > Restrictions > Deleting Apps isn't set 'off'

If you don't know the restrictions code then do you know whether it was on the iPad when you last backed up ? If not then you can restore to that backup and it should be removed. If it was on the iPad when you last backed up then the only way to remove it is to reset the iPad back to factory defaults and you can then re-sync your content back to the iPad - you won't be able to restore to the backup as that will keep the code in place.

Nov 26, 2013 10:37 AM in response to Phil0124by MEve77,I'm trying to completely remove a deficient app in order to redownload it (game I love). The creators of the app can't seem to figure out why mine is acting up. I delete it, but then it redownloads and get the same bug (the app just won't open, it goes black and then the icon screen appears). So I thought if my device can completely forget that I ever purchased it, I can purchase it again, hopefully without the bug.
By now I'm starting to wonder if it's a problem with my iPad.
