Mac OS XIt sounds as if your battery needs to be replaced, just as a start. Hold down the option key and under the Apple icon in the menu bar and select "System Information..." and look at Hardware->Battery Information>Health Information>Condition and see what it says.
But your battery may be just a symptom of other problems... the best thing to do would be to take your machine to your local Apple Store/AASP and let them run full diagnostics on the machine.
Unfortunately, a SMC reset doesn't fix everything...
