Potential causes are myriad. Sometimes it's a badly behaved (often 3rd party) driver locking up some critical I/O. Sometimes it's running out of memory. Sometimes it's hard drive corruption. Sometimes it's badly written software tying up system resources in one way or another. The list goes on.
Activity Monitor may be able to shed some light on what is happening. You could look for unusually high CPU, disk, or memory swapping activity. You could also get info on specific stalled applications to see what they are up to. Are they using a lot of CPU? Are they thrashing memory? If you hit Sample, you may get some hint about where the application is stuck from the function call stack.
The system log may also shed some light on persistent problems. You can view it through the Console utility.

Back up your documents and everything else important to you before proceeding. It is premature to conclude your hard disk is failing, but the symptoms you describe may presage an impending disk failure, and you should have backups anyway.
To help determine the possible causes of this behaviour read the following. None of what follows is intended to fix anything, but it will provide the additional information required to advance troubleshooting:
Boot OS X Recovery by holding ⌘ and r (two fingers) while you start your Mac. At the Mac OS X Utilities screen, select Disk Utility. Select your startup volume (usually named "Macintosh HD") and click the Repair Disk button. Describe any errors it reports in red.
When it finishes restart your Mac and test again for operation. If it's still not behaving as you expect it should, please post the results of EtreCheck in accordance with the following instructions:
Apple Support Communities contributor etresoft wrote a very useful app to quickly gather certain system information that may help point to a cause of this problem. Go to his website, download and run EtreCheck:

EtreCheck was designed to remove any personal information (such as your computer's name and serial numbers) but if you see anything that looks like an email address or any other personal information that should not be divulged to others, please delete or obscure that information when you post the reply.
When you are finished with EtreCheck, quit the program. It occupies very little space, and you can keep it or drag it to the Trash as you wish.
After completing the above please determine if the problems also occur in "Safe Mode":
- Safe Mode or "Safe Boot" is a troubleshooting mode that bypasses all third party system extensions and loads only required system components. Read about it: Starting up in Safe Mode
- You must disable FileVault before you can start your Mac in Safe Mode.
- Starting your Mac in Safe Mode will take longer than usual, graphics will not render smoothly, audio is disabled on some Macs, and some programs (iTunes for example) may not work at all.
- Merely starting your Mac in Safe Mode is not intended to resolve the problem, it's to observe its performance without certain additional components.
- To end Safe Mode restart your Mac normally. Shutdown will take longer as well.
System performance problems for reasons that cannot be isolated to any other cause justify an SMC reset. Be sure to read the procedure carefully and follow all the steps exactly as written, even if they seem inapplicable or trivial.

I apprecieate the response, however, I have used Activity Monitor extensively and noticed that the two biggest culprits may be "QTKitserver" and "deamon" processes spawned by "luanchd", such as "sh", "sed", and others.
John:
My wife's computer was the first to have issues withthe SWOD and we took it to the "Genius" bar, they concluded as you did. So, we replaced memory and hard drive, and upon minutes of restarting, it had the SWOD. I don't think in our case it's hardware.
However, I do believe it is buggy code. I also think that the OS X does not do a good job of trapping the bad code and gracefully exiting the situtation; ie, it leaves you with the SWOD.
I hope someone knows how to get around this. It is costing all of us a lot of time that we cannot recover from.
Help is needed here.

John's advice should suggest where the problem lies, since although I'm sure you're right about AM showing certain things using cpu, that isn't usually the case.
If your Apple store visit led to known good replacement memory & installing a standard OS X on a new empty drive without migrating/transferring any software settings, & yet within minutes there were problems... then it's hard to believe that anything other than hardware could be at fault, or surely most everyone would have similar issues.

Dec 27, 2013 4:17 AM in response to cwgloverby MrHoffman,"My wife's computer was the first to have issues.." So you have two entirely separate OS X systems (both presumably running 10.7.5?), and both with similar "beachball" wait cursor symptoms?
If so, that can point to a common trigger. A trigger that's probably not local hardware; double hardware failures can and do happen, but they're not common. (Full disclosure: I have seen similar systems all have similar failures, when a batch of systems were purchased together and a common component was malfunctioning; a bad batch of widgets was used in all of the systems. It happens, but it's quite rare. Particularly rare if the systems were not acquired from the same source and at the same time; if the systems lack nearby serial numbers and all.)
While it's certainly possible that the trigger is OS X, it would be fairly unsual for this "beachball" to show up at just one site and not everywhere that's running 10.7.5. We'd usually be hearing about "beachballs" everywhere, if the problem were generic to OS X, or to a particular series of MacBook running 10.7.5. (That sort of problem has happened in the past, but rarely.) The lack of "beachball" chatter would tend to imply there's something else involved here. Some other factor, or some else needed to trigger the "beachball".
Potentially some software that is installed on both, or maybe a problem with the local network connection.
I'm going to guess you're connected via that absolutely marvelous gremlin farm known as WiFi, so please try switching over to a wired connection and see if that makes a difference. WiFi can be subject to interference, and the upstream networks can be subject to both configuration and wiring problems.
Also (as a guess) see if specifying your DNS server as 8.8.8.8 and 8.8.4.4 and not whatever your ISP has set makes any difference; this via System Preferences > Network > select your network controller > Advanced > DNS. The 8.8.8.8 and 8.8.4.4 IP addresses are Google (public) DNS servers, and they can provide a reasonable way to test with DNS servers other than those provided by your ISP. DNS errors can present as network retries and delays and can trigger various "beachballs" in applications.
One of the other potential triggers for "beachballs" that can specifically awaken QTKitServer are shared (and corrupt) video files, and there have been cases where QTKitServer becomes overly active when there are corrupt video files around. I don't know if that's been seen on 10.7.5, but it has been seen before. (I would not expect this to arise on both systems if one's been wiped and reinstalled and no files are present, though. That points to the network or some other common factor.)
etrecheck can help find problematic browser extensions and related, plugins for video viewing, and should also flag anti-malware tools and other potential triggers; virus scanners can particularly cause virus-like behavior. Do please post that output.

We bought it home and used Time Machine to restore her apps and work. Then we got the recurrence of the SWOD. It was not a clean hard drive.

Dec 27, 2013 10:24 PM in response to cwgloverby John Galt,Your system has been so extensively modified with "anti-virus" garbage and ill-conceived "cleaning" apps that normal operation cannot reasonably be expected. In cases such as this an "erase and reinstall" is justified in which your system is completely erased, OS X reinstalled, and your essential files restored from a backup, omitting the abundance of useless junk contributing to its poor performance.
If you are not willing to perform that you may wish to consider uninstalling several modifications known to cause problems themselves. The aggregate effect of having installed so many will cause unpredictable results.
- Uninstall any and all Symantec / Norton products according to its instructions. Ensure all of its components are completely eradicated from your Mac. Your Mac cannot operate as designed using such products.
- Both MacKeeper and CleanMyMac2 belong to a broad category of time- and money-wasters capable of causing system corruption that can only be rectified by reinstalling OS X, restoring from a backup, or completely erasing your system and rebuilding it from the ground up.
- Follow these instructions to uninstall MacKeeper. They have been tested with the most recent version of MacKeeper (v 2.8) and last year's version. Earlier versions than the one released in 2012 require more extensive work to uninstall all its components.
- If you used MacKeeper to encrypt any files or folders, use MacKeeper to un-encrypt them first.
- Quit the MacKeeper app if it is running.
- Open your Applications folder: Using the Finder's Go menu, select Applications.
- Drag the MacKeeper icon from your Applications folder (not the Dock) to the Trash.
You will be asked to authenticate (twice):
You do not need to provide a reason for uninstalling it:
- Just Click the Uninstall MacKeeper button. You will be asked to authenticate again.
After it uninstalls you may empty the Trash and restart your Mac. All that will remain is an inert log file that does nothing but occupy space on your hard disk.
Next, uninstall CleanMyMac:
According to its web site CleanMyMac2 is supposed to uninstall itself:
"The easiest way to completely uninstall CleanMyMac 2 is by using CleanMyMac 2 on itself.
To remove CleanMyMac 2 from your Mac:
- Open CleanMyMac 2.
- Proceed to the Uninstaller module.
- Find CleanMyMac 2 in the list of apps found by the module.
- Click Complete Removal in the Smart Selector panel to mark the application file and all it related items for removal.
- Click Uninstall.
- Confirm that you are going to remove the app in the newly appeared message.
That's it!"
Apparently it may not be that simple though, as at least one user reported that it did not work, and required manual removal: Extreme Battery Drainage
Other problematic system modifications exist:
- Google Drive and other invasive Google system modifications are present. Google has yet to demonstrate the ability to write apps for OS X that do not cause performance problems. If you decide to retain them, you must accept the resulting degradation in system performance, battery life, and overall stability.
Uninstall them and evaluate the results.

I suggest you partition your hd into three parttions:
1) clean system with no add-ons: zero plugins and zero preference panes. When your system goes haywire, you can boot this clean system and see how things are going.
2) Normal production system. minimize plugins & preference panes. Get good reviews of the those that you install
3) Playpen for all the intreging things you find on the internet. At present, your system is in state three.

I'll start with just an example of what I've been facing. During the composing of this post, when I switched from Firefox to Word, I had to wait 60 seconds before the spinning wheel stopped and I got a cursor. I then copied one set of Etrecheck results, switched back to Firefox, hit Command-v to paste and had to wait 45 seconds or so for the spinning wheel to stop. And the paste operation didn't paste until I hit Command-v again (with no wait this time).
Here's what I've done:
I tried three times to run a scan with avast! and all three times the scan was unable to complete.
I restarted holding down Command-r and it found and fixed many permissions, but found no problems with files.
I ran Etrecheck in a "Safe Boot" and in a normal startup and there were two things that were tagged as "[failed]" in the normal startup:
