Q: Lost my name on my Apple ID

I gave my daughter an iPhone 4 for Christmas. After much confusion getting her set up and trying to get her an apple ID (she's not yet 13 so we tried to sign her up using my info.) my apple ID is now in her name. How do I get my name back on the apple ID?

iPhone 5s, iOS 7.0.4
