There is no way to permanently delete anything from your purchase history.  However you can "hide" purchases.

You cannot permanently delete apps from your purchases history - ever. Your purchases are tied to your Apple ID and will always be tied to the ID. You can hide the apps but you have to do this in iTunes on your computer. If you hide them, you will not be able to see them in the purchased tab of the App Store.

    VERY VERY Frustrating....

I, also, want to perminantly delete an APP from my iPhone AND iTunes.

Hiding an APP is not the same as deleting and I want to delete :-(

I used to be able to delete in older versions of iTunes :-(

    Thoughts ? Advice ?

Imagine the inconvenience and amount of problems that would be created for iTunes support because users "accidentally" deleted something in their purchase history.  They would spend all of their time re-granting those purchases.  The hide option allows you to ignore it.  You can permanently delete an APP from your device but not from the purchase history of your account.  At some point, you wanted the item.  Delete from your device and hide from your library.  That's all you need to do.
