Q: Why TOC go first?

I have a Word doc with a TOC.  I'd like to create an epub version of this doc.  Basically, the files below are a small sample of the same scenario I'm trying to fix for a book.

The TOC always goes first when the epub is created.  I haven't been able to figure out why it doesn't follow in the same order as the Word doc.  My workflow is to open the Word doc in Pages v5.1 and export as epub.  You'll notice three things happen in the epub:

1.) TOC moves to the front

2.) TOC is bolded

3.) Title of TOC ("Contents") is on a page by itself

I'm opening the epub in iBooks on my Mac.  Anyone know why this happens or how to fix it?

Here's the three related files:

    Hi, I don't know about creating ePubs with Pages, but; if you expand the ePub, open epb.opf archive, you have this section:

    <spine toc="ncx">

    <itemref idref="toc" linear="yes"/>

    <itemref idref="cover" linear="yes"/>

    <itemref idref="chapter-1" linear="yes"/>

    </spine>

    This is the linear reading order. If you change for this:

    <spine toc="ncx">

    <itemref idref="cover" linear="yes"/>

    <itemref idref="toc" linear="yes"/>

    <itemref idref="chapter-1" linear="yes"/>

    </spine>

    The cover will be the begining of the book.

    You can download correct ePub:
