It sounds like you are not seeing the options to reset your security questions or send an email to your rescue email address. If you have not setup a rescue email in the past or do not see the option to send an email to your rescue email, you will need to contact iTunes Support to reset your security questions:
If you decide not to provide a rescue email address, you can't reset your security questions and answers yourself later in the event you can't remember the answers to your questions. You'll need to contact iTunes Store support to have your questions and answers reset.
