Not sure how the decoupling came about but i'm thinking it might be because at one point i had my phone backing up to iCloud...that's just a speculation. did you ever back up to iCloud?
in any case, the steps i took to rectify the situation were:
1) back up phone
2) in iTunes, uncheck the categories of things (eg. books, music, tv shows) being synced
3) from phone look to see what's taking up space that iTunes is responsible for... to do this go: settings -> general -> usage -> storage
4) delete the stuff on there you don't want sitting on there...in my case it was some audiobooks in the music section...(i think they were there from ages ago - i had unchecked them in iTunes some time back)
5) go back to iTunes, re-check the categories of things (e.g. books, tv shows etc) you want synced.
6) sync iphone
- you might be able to just go into the usage folder yourself and identify stuff in there that is not accounted for by iTunes...but i think the method above is less tedious.

In iTunes no categories are checked/synced.
My music is on iCloud.
I have photos and I was thinking that this may be the problem. I have therefore disabled photo stream but the gap between phone storage and iTunes is still there. The difference is around 2 GB.
I also have 1.9 GB of Other field, visible on iTunes.
After 2 years of usage I feel like my iPhone 5s needs a hard reset but I really do not understand why this issue is there.

I did a hard reset, but restore the phone with previous backup in order to get my info. So this doesn't work.
I feel like doing a hard reboot and doing a fresh sync, but i don't want to loose all the info.
Please HELP!!!

Aug 31, 2015 7:12 AM in response to gym1champby mateofd,Same problem with my iPhone 6. It says that the storage is full and iTunes says I should have 1 or 2 gigs free. I have restored several times and everything works fine for a few days/weeks.

Sep 2, 2015 7:42 PM in response to gym1champby Technology4life,I also have this same problem on my 6 Plus. I've done a hard reset and I really dont want to reset to factory. I think the problem may lie in the applications though, any ideas? Perhaps clearing up the app cache of each app somehow

My Music is stored in the cloud, photos as well and my phone tells I have 5.4GB left while iTunes says 8.54GB. what the...???? I have uninstalled Facebook, Messenger, Instagram and other social media apps (apps which store a lot of data) then reinstalled them because I have nothing to lose by doing it. but still there is a huge free space difference. I don't understand why, but at least I found this article so I know I'm not the only one experiencing this.
If anyone finds a solution, please do share with us!
