Welcome to Apple Support Communities.
It sounds like AssistiveTouch is enabled on your iPhone and you're running into an issue disabling it. The article linked below provides more information about AssistiveTouch and will walk you through turning it off.

You can also disable it with iTunes, as shown in this article:

Managing accessibility features using iTunes
- Connect your iPhone, iPad, or iPod touch to any computer with iTunes installed.
- In iTunes, select your device.
- From the Summary pane, click Configure Universal Access in the Options section at the bottom.
- Select the feature you would like to use and click OK.

Oct 28, 2015 3:37 AM in response to Jason Lby William Donelson,However, you cannot use iTunes if you cannot enter the PASSCODE after restarting your iPhone. (Due to idiot setting up Assistive Touch with Double-Tap always on)
Thus you cannot use iTunes to Configure Universal Access.
