How can I get my iphone to add automatically a 1 at the beginning of all numbers.

1-415 234-4532 rather than 415 234-4532

If you are an AT&T, Verizon, or Sprint customer in the U.S., then do:
Settings > Phone > Dial Assist = "On".

If you are in the US you don't need the "1" region code prefix to call a number. Just the 10 digits.

I am in Canada and all long distance numbers need a 1 as a prefix. I don't want to go manually in and add the 1 to each of my contacts.

Try a call without the 1. I'm pretty sure I've called from Canada without it.
