If so, only the phone and Applecare bought at the same time would be covered. They should be able to link it via the serial number of the machine.

No... I just said that because I have a new iPhone 6 now, that's why the iPhone 5 is now my old iPhone , I bought the iphone 5 and the Applecare at the same time, but when I've checked the information of my devices in apple web page, there's no reference to it in my iPhone 5 information.

This is an Apple document covering your situation. There are others so you might google the question 'How do I check my Applecare agreement for an iPhone5.' It comes up with half a dozen. This one has US within the URL so may not apply to your country.
