On a single-disk Mac, more than 4 partitions are not normally allowed, unless manually customized. You have 5 entries (GPT 1-5) as shown in your GPT output.

You need to create custom MBR for this to work. Please post the output of

    sudo dd if=/dev/rdisk0s5 count=1 2>/dev/null | hexdump -C

This will show the first block of partition from your Bootcamp partition.

How was Windows and Data and OS X created/installed?

I installed windows via bootcamp in maverick and make it available virtually in Parallel Dekstop, there is no problem whether i ran windows standalone or virtually on Maverick, but then i did clean install Yosemite from USB because my MB stuck when trying to upgrade to Yosemite.

What is the year/model of you Mac? Please see How to identify MacBook Pro models - Apple Support. Do not post your serial number on the ASC.

    1. You Mac does not support EFI Windows properly. Was it used to install Windows?

    2. Did you manually create Data partition before BOOTCAMP was installed?

Yes, I was manually created Data partition before installing Windows with BCA, everything is fine until I did clean install to Yosemite

The entry between GPT4 and GPT5 is usually created when Windows is installed using EFI Boot (it is 128 MB) and it is supposed to be a Microsoft Reserved Partition. The recovery of your data is partially dependent on this answer.
