Another thing you can do is edit the source list and tap the checkmark as shown here (at the bottom):

Re-install the app (if not still installed).
Hold the screen to get the delete option. If you delete the app itself (I did 'remove data' but it shouldn't matter), the 2nd prompt should be to delete the data from the Health app. If you delete the health app data, it will depopulate as a source inside Health.
