I think the the reason for this is that - after first use, the system process that uses the file changes it. The list of safe to ignore has been growing with each new OS X release.
(Repair permissions is based on the fact that all files/apps/etc that are installed by the Apple Installer leave a receipt on your Mac. The receipt contains a list called BOM - Bill of Materials. The Bill of Materials contains the "initial" permissions settings for each file installed.)
