It happens automatically once you pair your watch (not sure the frequency, but it's pretty often).
You can tell by opening the Health app on the phone and see the updated values such as HR, which wouldn't have been possible before the watch unless you had another app reading a HR monitor.

I had the same problem. I just realised you have to select what health data you want to be shown on the health app dashboard. To do this go to the health data tab in the health app on your iPhone and tap all. From there you can select what data you want to appear on the dashboard.
