Q: can't seem to setup under 13 iTunes account

In order to use Family Sharing that includes a child account (in the USA this means someone under 13 years old) the family organizer must use a credit card as the payment method. See this article for reference -

Note that after the account has been created you can change the payment method back to a debit card if you wish.
