Make sure to transfer everything from your old computer to your new one first, as the current iPhone content will be replaced by content from the new computer.

The music sync is one way - computer to iPhone. The only exception is iTunes purchases.

If you connect it to a different library it will erase the phone and sync the new library's content. If you do not want this to happen, copy the library from the PC to the Mac so the phone sees the same library when connected that it did before.
Moving a library: Quick answer if you use iTunes' default preferences settings: Copy the entire iTunes folder (and in doing so all its sub-folders and files) intact to the other drive (often aided using an external hard drive). Hold down the option (alt) key (shift on Windows) and open iTunes. At the prompt to create or choose a library choose the copied iTunes folder.
