Q: HD 4000 Problems

Problem:

My MacBook Pro started showing graphic issues. For a little bit more than two years the system was running without any major issues, but now it is almost unusable. Last month a few of those hiccups started to occur, but in the last few days they happen more frequently (today over 50 times).

The graphic issues manifest in stuttering user interface, which usually means that the monitor freezes and temporarily becomes unresponsive. Sometimes the a second occurring freeze after 10 sec results in an application crash, sometimes the applications run a little bit longer. Sometimes the stutters throw me back to the log-in screen or restart the computer.

This behavior occurs in applications such as Safari, Preview, QuickTime, VLC and Heroes of the Storm. It's also impossible to play videos.

What I already tried:

The extended Apple Diagnostics tool didn't show any errors. I've reset PRAM and SMC. I also reinstalled the Combo Update 10.10.4 and I even reinstalled the OS from the Recovery Utility. The error persists. I have attached the beginning of one of the console logs which is related to the freezes.

Any other ideas on what I can try?

Unfortunately, sounds like a hardware issue.  Make sure your firmware is up to date. About EFI and SMC firmware updates for Intel-based Mac computers - Apple Support

Otherwise you likely need to contact Apple if it is a hardware fault.

EFI and SMC are up-to date.

I observed that when the UI freezes you can still move the mouse.

So it looks like either you have a software glitch that has broken your video driver or you have a hardware problem with the video chipsets.  If it is hardware there is nothing you can do except go to Apple for repair.

Most logical way to determine if it is a hardware problem or not is to do the following:

    1. BACKUP BACKUP BACKUP
      • TimeMachine
      • Clone the disk, using target disk mode if you can
    2. Perform a complete clean Yosemite installation, re-partition the whole disk, deleting everything.
    3. Make sure everything is up-to-date
    4. Before you install any Applications run it for some time to see if the video issues occur
    5. If the video problems reoccur then you have a hardware problem, contact Apple Support

    Alternatives but not logically proof...

    1. Backup Backup Backup!
    2. Run a disk and permissions repair from a recovery partition or bootable thumb drive
    3. Install the combo update OS X Yosemite 10.10.4 Combo Update
    4. If that doesn't work, re-install OS X on top of your existing installation

You could still end up with a corrupted file, driver, etc.  That is why the full clean installation method is ideal.
