Jul 5, 2015 10:32 PM in response to rraffby Rysz,Disconnect your iPhone.
Restart your computer and iTunes.
Reset iPhone by holding down the Wake/Sleep and Home buttons until the Apple logo appears.
Try connecting and syncing again.

Jul 6, 2015 11:12 PM in response to Ryszby rraff,Partially helped in a strange way, (I did also turn off iCloud Music Library)
The iPhone now "Syncs" in the sense from iTunes it updated playlists, could add or remove apps, transferred purchases back to iMac
BUT
strangely did this while displaying the same error message (although clearly phone was connected as it was doing the above, and went thru the various steps for backing up)
AND
it did NOT change the date of the latest backup (so Apply changes for instance from the Apps tab worked but not the Back Up Now button just eventually generates that iPhone disconnected error message)
My iPad mini that is still using 8.3 updated and backed up fine
thanks for helping in case you have other ideas.

Sep 14, 2016 11:19 AM in response to Ryszby MtnBiker,Worked for me. I'm sure I must have tried this in the past as well as numerous other restarts of computers and phone. Maybe iTunes 12.5.1 fixed it which I had just installed before trying this. In any case I was able to backup after maybe a year of not being able to.
