TCP 5223
TCP 80
TCP 443
TCP 3689
TCP 8000-8999
TCP 42000-42999
In my case, (a Suddenlink cable subscription with about 20Mbit down and 1Mbit up), 200kBytes/sec seems to be the maximum speed I can get before step 3 fails.
The upload is pretty slow, so I tethered via my iPhone's 4G connection for the bulk of the upload, and now iTunes Match only needs to upoad new CDs I rip and similar updates. 200kBps isn't bad for 10 songs at a time.

I couldn't get Entonnior working but did get Apple's Network Link Conditioner working.
Was able to get Genius turned on pretty quickly. itunes Match was stalling at 'Sending your results to Apple'. Stopped the process > drop down menu 'Store' > 'Update Itunes Match' got the whole thing working again.
Now I'm going to catch up on some sleep.
