I think this means you see the small digital touch button at the bottom of that person's contact screen (finger pointing up).
Press that button, then press the "i" on the lower right for help and practice.

I can draw patterns and I can do the 2 finger press to show my heart beat.
However it does not get sent to my contact.
I also do not see an "i" on the lower right.

The User Guide (above) references the "i" although admittedly I do not see this referenced in the Apple Support document referenced in my previous response.
I personally do not know anyone else with an Apple Watch so I have not tried this out myself.

However that "i" is simply not there on mine watch but is shown on my wifes.
When I check that on my wifes all the "i" does is show the 3 different touch options.
Doesn't really give any insight as to why neither of us can feel the touch
Any other ideas?

On each iPhone, check that the contact record (in the Contacts app) for the other Apple Watch owner includes that person's Apple ID email address.
You can then also try restarting each Apple Watch:
Restart Apple Watch: Press and hold the side button until you see the Power Off slider; Drag the slider to turn off completely; Afterwards, press and hold the side button until you see the Apple logo.
