Unlocking your iPhone is, indeed, a process that your carrier will need to undertake (this would be the carrier that the phone was last activated with). The article linked below provides further information regarding the unlocking process. I've also attached an article that details where to find your iPhone's IMEI and Serial Numbers if you've backed it up in iTunes.

No, Apple cannot unlock phones. Only O2. Whoever you spoke to is giving you a line. What is possible is that Apple did not notify O2 that they exchanged your phone, so you might have success asking Apple to do this. But more likely Apple DID notify O2 and they just didn't update their system.
