Aug 15, 2015 5:58 PMby elcpu,In my opinion you have a hardware issue, specially after the steps you have taken. If the phone is still in warranty which it likely is, I would take it to an Apple Store for a test and replacement. I would make an appointment with the Genius Bar over the phone or web. Good luck.
