Q: how do I un-sync my iphone with itunes

Holy guacamole !!!!! - so sorry for posting what should be straightforward issue and appears as a hardcore dumb newbie question.
Here's what I have and need help with:
I have an old Windows XP machine, that I have rebuilt numerous times (hard drives, graphics cards, etc.) which for now it only exists for old Flight Sim games. My issue is this, on this Windows XP machine I have iTunes 11.4 running where I have a ton of music files and ring tones, etc. I purchased and added from my personal CD's. For the umptenth time I re-loaded Windows XP after replacing some hard drives. I need help understanding how I 'un-sync' my iPhone 6 from this Windows machine, so I can sync with my Macbook Pro (which I love!!!!). I want to just sync my iPhone 6 with my MBP - and not the old Windows XP machine
My goal here is this, I want to sync my iPhone 6 with my MBP where I have it running iTunes running just fine, I need all the ringtones and music I purchased from the old Windows XP to appear (transferred) and be present with my iTunes library running on my MBP.
How for the love of God can I do this ? I have tried a few things and then aborted when it looked 'jacked' - I mean can I not just grab the iTunes folder on the Windows machine and merge (somehow) with the Apple machine and all be copasetic ? I just, just want all and everything from the Windows machine I purchased since 2004 to appear and show up in the Apple MBP ? Please to help ?
