My stupid Iphones (5 and now the 6) have been doing this for years.  I just gave up, and no other "fixes" I found on line worked until this one.

HoustonK's suggestion helped because it led me to a solution.  On my iPhone 4, however, when I unmuted the bell using the Assistivce Touch>Device>Mute icon, oddly enough when it shows the phone is not muted, it won't ring.  I thought I would cycle it through Mute>Unmute>Mute>Unmute to see if it would then Unmute, but what I found much to my surprise, is that it rings when the Mute/Unmute icon is set to Mute and it doesn't ring when it's set to Unmute.  Talk about counterintuitive!  Looks to me as though there's a glitch in the software.

This actually worked.....only difference is, my phone works with the "mute" button "on"  instead of "Off"........Go figure?     Apple would have probably charged me a fortune for this advice...........thankyou for your help...much appreciated.....
