    My guess is

    1. You upgraded to ELC.

    2. You can no longer boot into Windows.

    3. You would like some help in getting this fixed.

    Is that correct?

The gap between GPT3 and GPT4 indicates that you have a missing/lost NTFS header somewhere in gap.

Can you use either of the following links as reference and run Testdisk scan

Lost bootcamp after using mini tool partition to increase windows partition size

Bootcamp fails after Yosemite upgrade (Plz help me Loner!)

    Disk /dev/rdisk0 - 256 GB / 238 GiB - 500118192 sectors (RO)

    The harddisk (256 GB / 238 GiB) seems too small! (< 311 GB / 289 GiB)

Check the harddisk size: HD jumpers settings, BIOS detection...

The following partitions can't be recovered:

This is a list of unrecoverable partitions. Press Return/Enter to Continue and post the list of recoverable partitions.
