If you're using upgraded iCloud notes on IOS9, your notes aren’t visible on devices that use OS X Yosemite or earlier. I have just upgraded my mac from Yosemite to El Capitan and now the notes sync perfectly.
If I'm honest when I upgraded my iPhone and iPad I got some sort of message relating to this and ignored it (regretfully).
I think it was a timing issue as in IOS9 was ready for iPad and iPhone before El capitan was ready for Mac, either way when you do as I describe above everything syncs notes wise just as before IOS9. El Capitain is the answer and seems pretty good so far

I've stored all notes in a folder...then erased all in notes in outlook, and then erased in the iphone.
The sync (with zero notes) and then the sync is ok.
I drag and drop the notes back agin to outlook, and then sync again...and finally goes fine ('till iOS 10 or iTunes 13, I'm affraid...)
Not a very "high tech" solution...but a solution at the end...

I'm the one who solved the issue by erasing and copy the notes again...
It worked...but for my surprise I've seen today that, without any reason, it is having problems again with the sync of notes.
So...
Apple people...is anybody reading this?...or this is a scream in the desert? (and "Apple" style scream...).
I though Apple was better...mmm....thinking of mobile phone for future...it has good looking...

Oct 11, 2015 8:39 AM in response to johnfromkelsoby graeme b fraser,I use a PC and have exactly the same problem, I tried following the solution a week ago ie delete, sync, add to Outlook and sync again but no succes. As there were no other solutions proposed I tried again and it worked, so thanks but what a pain in the butt. Not a good show from iPhone.

After installing iOS9 the notes were run together in one big paragraph rather than each entry starting on its own line. Now they are back like they are supposed to be.

Oct 11, 2015 10:14 PM in response to pepeidolioquintanillaby Nonhominem,I also resolved it initially the same way "pepeidolioquintanilla" did. My environment is a Windows7 Ultimate Edition, MS-Office/Outlook 2007, iTunes 12.3.0.44, iPhone6 Plus with iOS9.
Like "pepeidolioquintanilla", I am now experiencing the same problem all over again. iTunes is duplicating Notes at random, failing to synch changes at random, fails to synch deletions ... it's a total disaster. After using my iPhones and Outlook for Notes for the last three or four years, this could be a deal breaker for me and I might switch to an Android phone if Apple is going to continue with the constant overburdening of bloatwear we don't need, while nerfing the apps we do need and doing nothing to fix it.
It's maddening how Apple has so completely lost focus on its consumer base and making these products work for us ... it's like they've become Microsoft and are building a dud-O/S every other release and just don't care ... when did that become who Apple was?

Hi, I have also synced notes with the Outlook Notes in my PC since iPhone 5 using iTune. It has worked very well until I upgrade to iOS 9.x. I also tried to follow your suggested solution to delete all the notes in both iPhone and Outlook. Then I copy back all 100+ notes back to Outlook and sync again. It did work initially. However, after a few times of sync, the error message "could not sync notes to iPhone because the iPhone disconnected" happened again. I noticed that there is a change to Notes in iPhone. They have added folder structure to Notes. I don't know if iTune cannot catch up this change. In the past, there was an option to check whether you want to sync notes in iTune Info page. Since iOS9.x, you never see this option in iTune Info page any more.
I rang Apple Support. They said they need to send to software engineer and asked me to see if the next version of iTune solved this problem.
