First try what ogelthorpe said, go to recovery mode (cmd - R) go to disk utility and erase disk. Choose format: OS X extended (journaled) and title: you can name whatever you like, for example, macintosh HD then press the button "Erase...". Then go to the top left corner, next to the apple log (), press on disk utility > quit disk utility and you will back to the main menu.
Then choose reinstall OS X (2nd choice) and follow the instructions, you will need your apple id and password.

Oct 17, 2015 7:40 AM in response to rowns1by OGELTHORPE,You said earlier that you had a 13" MBP with Intel 4000 graphics. If that is the case, you have a mid 2012 MBP that originally came with Lion installed.
Boot the MBP into Internet recovery, OPTION + COMMAND + R keys.
The display should show a revolving globe.
That will connect you to the Apple servers.
By following the directions, you will be able to reinstall Lion.
Note: Before installing Lion, format the HDD in Disk Utility>Erase to Mac OS Extended (Journaled).
A solid Internet connection will be required.

Oct 18, 2015 2:08 AM in response to OGELTHORPEby rowns1,I tried to format the HDD like you said, I have waited the whole night to let him finish it, when I looked today it said It was going to take another 14 hours. probably still increasing. Also my hard drive doesn\t show up anymore in the list.

Oct 18, 2015 3:12 AM in response to rowns1by OGELTHORPE,Kaz-k may very well be correct, the HDD is faulty. You can gave the MBP checked out at an Apple store genius bar for free and they certainly can determine the health of the HDD. Or install a new one which is what I would do.
