It sounds like you have forgotten you Apple ID log in and cannot continue past the activation screen, or you cannot remember your iTunes account password. The steps for both are essentially the same, you will need to reset your password to regain access to your account or device:
If you forgot your Apple ID password
If your device is disabled due to entering the Passcode too many times incorrectly then use this article to resolve that issue instead:
If you forgot the passcode for your iPhone, iPad, or iPod touch, or your device is disabled
If this is not the case can you elaborate on what it is you are seeing, and what exactly is disabled?
