Oct 27, 2015 11:28 AM in response to Walmoby Johnbot5k,I am having the same issues...
I have done all the above "fixes"
and it's around and around I go...
A fix needs to happen - this is getting a bit painful.

Nov 7, 2015 1:50 PM in response to Walmoby fr33wh33lin,So my "solution" was to make a bootable Yosemite installer on USB stick, wipe and reinstall. EVERYTHING works now and I am a much happier customer.

Oct 29, 2015 2:25 AM in response to Walmoby Matthias Transier,★HelpfulIt seems that this issue is (at least for me) caused by the fix CVE-2015-5943 (see https://support.apple.com/en-us/HT205375 at the bottom). If there is any active accessibility tool (see system settings, security, accessibility), the keychain prompt assumes the input is coming from an untrusted source and simply does not accept it (which can be seen in the logs). The same holds for remote connections via vnc://.

Oct 29, 2015 8:56 AM in response to Walmoby Dcapp3,So i got this fixed. Took a while but reset your login keychain.
Go to "Keychain Access"
Got To the file menu and select "Preferences"
Select "Reset my Default Keychain"
It took care of it for me.

Oct 30, 2015 3:26 AM in response to Matthias Transierby ejl888,Thx, disabling all tools in (system settings, security, accessibility) solved the problem with Office 2016 .

Oct 31, 2015 5:18 PM in response to ejl888by Alex_Mann,What exactly did you do to fix this problem? Everytime that I open a microsoft application (word, outlook, excel) it will not stop prompting for "allowing" access to my keychain. I cannot click always allow or even allow, so this is very frustrating.

Nov 2, 2015 9:18 AM in response to Walmoby josemaria_cr,The problem (in my case) concerns a new security feature for avoid the vulnerability CVE-2015-5943
And occurs that remote control soft (TeamViewer) are generating a 'synthetic click', so the keychain is not affected while you are clicking to allow codesig.

Nov 2, 2015 9:01 PM in response to Walmoby Timejudge,I seem to have stumbled upon a fix for this. First you need to go into system preferences, internet accounts, add another icloud account, delete your icloud account, add it again, then delete the alternate. It seems to have fixed the problem for me. Keychains and Icloud

Nov 6, 2015 10:04 AM in response to Matthias Transierby finchr,★HelpfulIn my case the problem was with MagicPrefs, a utility that lets you define additional mouse and trackpad gestures. Disabling MagicPrefs allows Keychain to see Allow/Allow All mouse clicks.

Nov 6, 2015 12:24 PM in response to finchrby sjpye,Mine appeared to be a combination of MagicPrefs and Karabiner. I hope MagicPrefs comes out with an updated version... I'm severely missing several of those gestures now.
-
Nov 6, 2015 1:16 PM in response to sjpyeby finchr,I don't think this will be a simple fix for MagicPrefs, since it works by intercepting real events and resending them after processing gestures.
A simple workaround is to enable the menubar icon so you can easily enable/disable MagicPrefs. When something pops up a keychain dialog, disable MagicPrefs, click Allow, then enable it.

Nov 7, 2015 3:28 PM in response to Walmoby geolangston,I had this same problem - could not view passwords in my personal keychain (of which there are many) - on an imac. The problem did not manifest on my macbook pro however. This was after an update from Mavericks to El Capitan. After typing in password for the said keychain in order to view passwords for individual entries in the said keychain things would just lock up with spinning wheel. The only way out was to hard reset or, better yet enter "deny" in the dialog. As it turns out this was all due to the "magicprefs" mouse-enhancement software that I installed a while back in order to implement a middle mouse click (since I am a long-time unix/linux user).
After trashing all the "magicprefs files", everything is back to normal, and I can live with native OSX workarounds. Still, I think the middle click of a 3-button mouse is invaluable. If you've ever had it you know what I'm talking about. Hopefully a future mac OS will make it possible.
Hope this helps anyone else with this problem!
