I would recommend taking it in for service to be professionally diagnosed. If the computer is older than an 2011, I'd recommend taking it to an AASP in your area. If you are not aware of any, click the following link to locate one.

WIth memory in the fans spin up, then half a tone and done.
Not sure if it's worth bringing to Apple. It's about 10 years old. I replaced the HD maybe 5 years ago. So no shame to it.
But paying $100 just for a diagnostic, then repair costs may not be economically wise.
