Q: USB bus freezing on Mac Pro late 2013

This problem has been plaguing me for almost 2 years.  Here is a link to a discussion to an old discussion about my problem, in which I thought I had finally fixed it, then the issue came back a month or so later ( Mac Pro Late 2013 USB problems / freezing ) .

Basically, I have intermittent USB bus freezes.  Sometimes once a week, sometimes 3-5 times a day.  Each freeze forces me to do a reboot.  I've had this happen right in the middle of work, which is infuriating.

As detailed in the old discussion thread, I tried installing a fresh OS, and it seemed to work for a spell, but in the end wasn't the solution.  Here's a list of common troubleshooting steps I've tried:

-New OS install after formatting HD

-Fresh user account with no 3rd party software installation

-pulling USB devices off the bus until I have nothing left but a keyboard and HD*  (I'll detail this more below)

-Resetting the PRAM, PMU, EFI and anything else I can think of.

-RAM tests

Since the USB bus was what was freezing, and the thunderbolt and BT buses were unaffected, I assumed it was a bad USB device.  So I've tried all manner of combinations of using USB hubs, not using hubs, removing non-essential USB devices etc. The only devices I had to have on, was a keyboard (but I've got 3 different ones, and have had bus crashes with all 3) and a G-Tech USB 3.0 HD.  The reason I am not really able to not remove the USB HD, is because my user folder is split between the built in 256MB SSD and the external USB drive (2TB).  I mean, I *could* boot without it attached, but it would be a very boring experience, because no relevant files would be available.  So I've been reticent to try that.

However, recently I got the idea that if I had the console open during a USB freeze, maybe I could capture some meaningful info.  I think I did, but when reading this log, I realize I'm at the limit of my knowledge.  It's a bit confusing to me.  My assumption is that the relevant info to the USB freeze is around 9/5/15 9:30am - 9:31am in the log.

Can someone with more knowledge than me please look at this log and tell me what their diagnosis would be?  If any additional info is needed, please ask and I'll be glad to post it.

I just noticed this in the log:

Which worries me.  But I've done several multi-hour tests of the RAM, and it always passes.  Do I have bad RAM? Is that the cause of my USB problems or simply a red herring?

    Try reseating the memory

That's a great suggestion, and one I've already tried twice.  The first time, I simply pulled it out, blew air on the sockets, reseated the DIMMs firmly.  The second time I even juggled them around, I.e. put DIMM 1 in DIMM 3's sockets etc.  Just to see if it would make any difference.

I think the ECC memory errors in the log (since correctable) are just a Red Herring.  I mean, I can accept that memory errors could cause a USB freeze, but I'm unable to accept that memory problems would ONLY cause USB freezes.  The system is otherwise stable.  I've never had a kernel panic in 2 years.  And once the USB freezes, my BT trackpad still functions, so I'm able to properly shut down from the OS menus.  Just seems very targeted. I don't believe bad RAM would be so targeted at one system.

Try rearrranging the memory. It could be that there is a memory problem that is not corrected and since it is also the same bit the same data) USB info) could always be loaded in the same place.

    If you open System Profiler

OS X: About System Information and System Profiler - Apple Support

and look under memory it will say OK if it found not correctable errors. The at info is generated when you open Profiler so you have to open Profiler each time to get a new snapshot.

ECC errors do not get better, just worse

Yu can try a fresh instal of Yosemite and only restore data. Reinstall apps and other things

    These are all good suggestions.

At this moment, system profiler states that all three DIMMs (12GB total) are "Status: OK."  Additionally, every time I've attempted to run Apple Diagnostics (D on start), the system checked out OK.  Also, I've installed Memtest, the command line tool.  I've booted into Single User Mode, and run memtest for many hours, always coming up with no errors.

I've run sys diagnostics just now, still says everything fine.  I'll run memtest again in a moment.

The suggestion to install the OS fresh was a good one too.  One that if you read my original thread from almost a year ago, I thought had solved my problem.  But the USB issues came back shortly.  Then I reformatted my drive and tried a fresh install again, and the problem was present the same day.  So I concluded that it had no effect on the issue.

I appreciate your time spent on this issue.

In the end, it was hardware.  Apple replaced the CPU riser card as well as the I/O back panel (which basically constitutes what a motherboard would be in a older Mac Pro).  No changes to the software at all, no peripheral changes, and now the machine behaves perfectly.
