    It's a security risk to post this personal information and I've asked the Hosts to remove it.

     

    Go to http://appleid.apple.com and click 'Find your Apple ID' and proceed from there.

     

    If this doesn't work for you, then provided you bought the phone new you can take it to an Apple Store (not an independent shop) together with the original purchase documents and they may be able to unlock it for you. if you bought the phone second hand they will not be able to do this.
