I am unsure if you are having an issues with the receiver or the microphone based off the information that you gave. Either way, you do have some troubleshooting options to get this sorted out.

The first will go over microphone troubleshooting on your iPhone. This will be good if the person you are calling can't hear you. The second will be the receiver and that is good for troubleshooting issues where you can't hear them.
