Q: pricing on a new iPhone 6S

If I buy an iPhone 6S with 64 GB the prices $749, payable at $37.45 a month.  They provide an annual upgrade. So in the second year am I still paying the $37.45 per month on the old phone and an additional amount on the new phone?  Howe does this work?  Thanks
