A receipt is sent to the email address for your iTunes account with the name of the app at $0, and the app is included with the purchase history for your iTunes account even though the app is free. This is recorded as a download for the app developer's records, and for Apple's records. Apple doesn't charge the app developer anything for hosting and distributing a free app but download records must be kept. Some free apps also include an in-app purchase.
