Apple Watch does not presently include a built-in blood glucose monitoring system (and no-one here can tell you anything about Apple's unannounced future plans).
It does, however, have a built-in heart rate sensor.

For example, Dexcom offers:

- The companion iPhone app for this product is described as supporting the display of information on Apple Watch:
- "Dexcom Share2 is compatible with your Apple Watch. You can view your own glucose information and trend graph right on your wrist."
