Q: Device removed from iCloud? Not authorized?

I traveled to Florida this past November and unfortunately it has taken me up until now, from November 14, 2015 to January 20, 2016 to notice that I more than likely didn't come home with my iPad mini (I don't use it that often). I'm assuming I either left it at a security checkpoint in the airport or in the backseat pocket on the airplane. Anyways, (in a weird twist), my iPhone crashed on me and that's when I started looking for my iPad so I could resume my conversations but couldn't find the iPad. I also remember not too long ago looking at the devices registered with my Apple ID and it said my serial number for my iPad was invalid and I would have to correct it. Well, I filled out the lost and found form for both the airline and the airport and I wasn't able to fill out the serial number for my iPad because now it looks like the device has been completely removed from my Apple ID.

The iPad is currently shut off so Find my iPhone cannot locate the device. There is also a 4 digit passcode on the device. Is it possible to factory reset all of these things if someone were to find it?

I understand that you have lost your iPad mini and have some questions. First, I’d like to mention that you can find your serial number for your iPad without having the device present:
If you don't have your product or it won't turn on

You can find your serial number on the surface of your product.

If you have your product's original packaging, you can check the barcode to find the serial number.

If you have an iPhone, iPad, iPod touch, or iPod, you can find the serial number from the Devices tab in iTunes preferences.

You can also find your product's serial number on the original product receipt or invoice.

To answer your question, your device may have Activation Lock enabled, which can prevent others from resetting the device:
