To set up the Activity app on your watch:
- When viewing the time on your watch, tap on the Activity rings complication;
- Or go the Home screen (via a single press of the Digital Crown) and tap on the Activity app icon (coloured rings).
- When you first open the app, you will need to enter some personal information.
Also:
- On your iPhone, go to: Settings > Privacy > Motion & Fitness - check that both Fitness Tracking and Health are enabled.
- On your iPhone, in the Watch app, go to: My Watch > Privacy > Motion & Fitness - check that Heart Rate and Fitness Tracking are both enabled.
- On your iPhone, in the Watch app, go to: My Watch > General > Wrist Detection - turn this on.
- If you have also enabled Passcode, unlock your watch after placing it on your wrist.

Jan 27, 2016 5:31 AM in response to Jonathan UKby cpdanville,I have done all the above steps, Jonathan. Each was already enabled. The correct wrist is noted as well in my Apple Watch settings in my iPhone. Any other suggestions?

On your iPhone, close all open apps:
- Double-click the Home button, then swipe up on each of the app previews to close them.
Restart both your iPhone and your watch:
- Turn both devices off together first, then restart your iPhone before restarting your watch:
- To turn your watch off: press and hold the side button until you see the Power Off slider; drag it to turn off;
- To turn it on: press and hold the side button until you see the Apple logo.
If the issue persists, it may help to unpair and re-pair your watch:
- The Watch app on your iPhone takes watch backups automatically, including a fresh one when unpairing via the app.
- Choose to restore your watch (Restore from Backup) when offered the option during set up. Most data and settings will be restored, with some exceptions (eg Apple Pay cards, passcode).

Jan 27, 2016 5:39 AM in response to cpdanvilleby Jonathan UK,Check also that you are wearing your watch snugly on the top of your wrist. With Wrist Detection and Passcode enabled, your watch will lock automatically if it believes that it has been removed from your wrist. A loose fitting band can trigger this by allowing the watch to temporarily lose contact with your skin.
Some wrist tattoos can also impact sensor performance, potentially affecting the Wrist Detection feature.

If the issue persists after trying all of the steps, I suggest that you contact Apple Support (mail-in service may be available) or make a Genius Bar reservation for further assistance and with a view to having your watch checked under warranty:

Jan 27, 2016 5:59 AM in response to Jonathan UKby cpdanville,Thank you for your time, Jonathan. I do not have tattoos and I do have it on snugly (though by now, I have tried snug, loose, and in-between). I guess I will have to make an appointment at Genius Bar.
The activity feature is what lured me to get the watch. My husband loves his.

One final step to try before making your Genius Bar reservation. Try force restarting both devices, iPhone first :
iPhone: press and hold both the Sleep/Wake and Home button for around 10 seconds, until you see the Apple logo.
Watch: press and hold both the side button and Digital Crown for around 10 seconds, until you see the Apple logo.
