Mac OS XIf all your devices are logged in to your Apple ID, go to your iPhone and under Settings go to iMessages - at the top iMessages should be selected and just below under Text Message Forwarding you should see two devices, Mac and iPad - make sure they are also both selected. If you only see one device try signing the missing one out of your Apple ID account and then back in.
When all devices are on the same account and you select Text Message Forwarding on iPhone your other devices should show a number that you have to enter to confirm the connection.
I've found that also for this option to work you need all the devices to be on the same wireless network, that also includes being on either 2.4 or 5Ghz (but not a mix!) - if your iPhone and iPad are on 5Ghz and your Mac on 2.4Ghz that could be the reason, try moving Mac on to 5Ghz or the other two to 2.4Ghz.

On the Mac version what tells you it has Stopped working ?
Have all Contacts you have tried go red and say "Not Registered with iMessages ?
Or when you send something is it immediately marked "Not Delivered"
If either of these are happening to you then the Mac is not logging in properly to the iMessages Servers.
You will need to Contact Apple Support and state you have an Apple ID issue (Free to fix).
Even then there can be a lot of hoop jumping for them to be convinced that it cannot be reset from your end.
Politely insist that the iMessages Servers is reset with regard to your Mac's use of the Apple ID.
Even at that point they are likely to tell you that it will take 24 hours to reset.
