Q: There's a passcode on a used iPhone at setup.

I have a used iPhone 6 + (iOS 9.2.1) that I got from a family member.  When the device was charged, I got the opening "Hello" screen, but it is locked with a passcode.  The device was supposed to have been restored to factory settings and erased, but there is a 6 digit passcode that is still on the phone that I am having difficulty getting around.  The passcode is not from the previous owner, as I tried using her previous passcode, which failed.  As I can't get into the device I can't use iCloud to erase it because I can't connect it to the internet or wifi.  I've tried using both recovery mode and restoring it in iTunes (I have Windows 8) but I keep getting error 3194.  I've tried various ways of fixing the host file but I keep running into problems.  Please help!
