For the Original iPhone you probably set the App Active on the iPhone and included the SMS option.
Later you will have activated the Apple ID on the Mac.
Then Added the Apple ID to the iPhone to Link the iPhone Number and Apple ID.
At iOS 8 and Yosemite you probably activated Text Forwarding (SMS) so your Mac can see them.
This requires that the iPhone and Mac are using the Same Apple ID AND are on the same LAN.
Is the new phone set up to do all this and is it on the same LAN as well.
It is worth remembering that the devices also use their Serial numbers in handshaking to "prove" they are the right device and swapping SIMs and using the same number with the Carrier are probably not enough.
Most likely you will have to reset from scratch.
